In medicine, digital sovereignty may become a patient-safety issue.

Singapore has launched SIMFONI, a national initiative adapting AI foundation models to its multi-ethnic population, local disease patterns, clinical guidelines and care pathways. Its first use cases address cardiometabolic decision support in primary care and multimodal AI for eye disease.

The important idea is not that every country must train a model from scratch. It is that medical AI should be clinically contextualised, locally validated and accountable to the health system in which it operates. SIMFONI remains an R&D initiative moving toward pilots, so its clinical impact is not yet established.
