A Smartphone Window on Brain Disease
A new npj Digital Medicine study moves remote smartphone cognitive assessment beyond simple feasibility. Researchers tested the Alzheimer’s Disease Digital Assessment Suite (AD-DAS) remotely for 28 days in 123 participants, including healthy controls, people with subjective cognitive decline and patients with early Alzheimer’s disease.
- 97.6% completed remote testing.
- 96% adherence was reported.
- Test–retest reliability was moderate to excellent.
- Performance differentiated early Alzheimer’s disease from the other groups.
- Several digital cognitive metrics correlated with brain atrophy in expected neuroanatomical regions.
My takeaway
That last finding is particularly important. A smartphone test becomes much more valuable when its signal begins to reflect underlying brain changes, beyond agreement with another cognitive score.
This moves us closer to digital biomarkers that are scalable, remotely deployable, repeatable over time and biologically anchored. Such measures could eventually support earlier identification, patient stratification and longitudinal monitoring.
This remains early validation. The cohort was relatively small. These measures are not yet established diagnostic biomarkers or substitutes for clinical, imaging or molecular assessments. Larger longitudinal studies must establish how well they predict future decline and support clinical decisions.
